To be used for passport applications processed by Belize:

Required  Documents For Apply for a Child Passport 

For passport applications processed within Belize:

• The completed application form must be completed and signed by one of the referees.
• Fill out the Recommender form 4A that is signed by a Justice of the Peace, Minister of Religion or Medical Professional, Notary Public, or Attorney in Law. It should have a 2×2 photograph attached to the form, with the following endorsement on the reverse “I swear that this is a genuine likeness to the (name of person) ” The form must be sealed and signed by the person who is the recommender. Make sure the person who recommends is able to stamp their name alongside the signature. The form should bear the cost of a stamp, which is BZ$1.50.
• Completed Recommender Form 4B that is signed by a Member of the House of Representatives (not a Senator appointed Minister) Chief Executive Officer Director of Department or a licensed teacher. A photo of 2×2 stapled to the form of recommendation bearing the following certificate on the back “I confirm that this is an authentic representation in (name that of person) ” then signed and stamped by the person who is the recommender. Make sure the recommender puts their stamp next to the signature, or in the case of the teacher, the licence number, or the school’s stamp. The form should be accompanied by the cost of a stamp, which is BZ$1.50.

• Form 4A for recommendation must be completed and is required to be completed by the recommender, a Justice of the Peace, Notary Public (USA/ROC), Solicitor (UK, Middle East) Minister of Religion or Medical Practitioner. A 2-by-2 photo stapled onto the form with the following certificate on the back “I confirm that this is a genuine likeness that of (name of person) ” then signed and stamped by the person who recommended the applicant.
• The applicant must show the minor’s Original Belize birth Certificate or the original Nationality Certificate.
• Original deed poll that was registered in Belize If the name of the person registered differs from the name on the birth certificate.
