Apply for Change of Name Ownership or Management of a Drug Shop In Uganda By Online

  • The applicant who wishes to change name, ownership or the management of a drug store is required to apply for permission through the National Drug Authority.
  1. Permission to change the name or Ownership of a Drug shop is available by contacting the National Drug Authority.
    • The management of a drug shops is controlled by the National Drug policy and Authority Act.
    • Any applicants who intend to file a request for change of Ownership, Name, or Management should be familiar with applicable laws and regulations in order to make sure they are in compliance.
    • The production, management prescription, importation, prescription and distribution of drugs in Uganda is supervised by the National Drug Authority.
  2. Complete the application form according to the instructions of the National Drug Authority.
    • The proposed changes should be accompanied by competent staff such as pharmacists, pharmacy technicians, or certified chemists.
  3. After filling out the form, complete the form and pay the fee that is prescribed of 100,000 Ugshs to the Bank in the name of the National Drug Authority.
  4. Fill out the form along with the payment slips to pharmacist The officer in charge of owner changes or name change in the area that the property is located or the nearby health district inspector office where officers will conduct an inspection of your premises.
    • NDA is not required to grant permission to transfer ownership of drug shops that have that are opened without adhering to the established procedure and is not accountable for any consequential financial loss.
  5. The applicant must have a supervising registered pharmacist working for the company prior to the application is made for a change in ownership, management, and name since this is a common practice in processing.
  6. Request your Permission to change the Name Ownership or Management of a shop the place where you submitted your application within two working days from the date your application was submitted to the National Drug Authority in case there isn’t a rejection.

Note:

  • It is the obligation of the person in charge and the owner/applicant for the drug shop to ensure that the certificate from the in-charge that are submitted to NDA is authentic and valid.

Required Documents For Apply for Change of Name Ownership or Management of a Drug Shop

All applicants to the Drug shop must fill out the following information by the 31st of January.

  1. Letter of clearance from Ministry of Health
  2. Previous approval from the National Drug Authority
  3. Proof of Services/business-registration/registering Drug shop Business.
  4. Original registration certificate (Business name)
  5. The drug certificate is verified through The National Drug Authority.
  6. Certificate of Incorporation (Company)
  7. A Proof Letter from Uganda Registration Services Bureau
  8. Prior Certificate of Suitability of the building
  9. The evidence that the drugs that are included in the ownership change are the essential Drugs Listed in Uganda.
  10. Pro-forma invoices for Drugs sent to Ministry of Health (MRU) is also required.
  11. Current Certificate of the full name as owner of the drug shop.
  12. A list of drugs and other items found in the drug shop.
  13. GMP certification (Good Manufacturing Practice (GMP) The certificate is to confirm that the shop complies with international standards for manufacturing medicines.)
  14. Certificate of Registration (this is a document that proves that the items in the shop have been registered and is used in this country.)
  15. Regulatory Unit (Ministry of Health) from the country.)
  16. License to operate an online or wholesale drug shop.
  17. Certificate of the Full Name of the Drug shop to serve as evidence of the legitimacy of business units.
  18. The filled application forms
  19. Letter of acceptance from the professional in charge plus a copy of his/her relevant Registration/Enrolment Certificate.
  20. A commitment letter from the officer in charge of supervising the dispenser.
  21. For retail/wholesale Drug store (human) A duplicate of the certification of the dispenser, or an approved registered or registered nurse.
  22. If there is a change in the drug shop, the applicant must present an official copy of the article and Memorandum of Association or Partnership Deed which lists the Drug store as one Directors or Partners or Partners.
  23. In the event of a change in ownership, the buyer has to provide evidence of the sale, such as copies of the sale agreement that the seller signed with buyer, as well as the most recent or updated memorandum of agreement and articles.
  24. Two passport photographs of the certified professional responsible.
  25. A copy of the certificate of the qualified medical/pharmaceutical /veterinary profession in charge.
  26. Previous approval by the National Drug Authority to Drug shop with a license to open in the closing year.
  27. Original registration certificate (Business name)
  28. Certificate of Incorporation (Company)
  29. Original lease and rent agreement from the Landlord for the business that is being established.

Eligibility

The applicant to be qualified must satisfy the following criteria:

  • You must have a “licensed Certificate of qualification for a drug store”;
  • The law is in place, and you must adhere to the professional code of ethics;
  • physical health, and can be maintained when working Drug Dispenser jobs;
  • Anyone who is 18 years old operating a Drug Shop business in Uganda with the exception of those exempted by Section 5(a) of the National Drug Authority policy and Authority Act are eligible for Drug Shop authorization to transfer the ownership.

Fees

Permission Fees 100,000 Ugshs.

Validity

Valid for one year.

Processing can take 2 days.
