  1. To notify to the National Insurance Scheme (NIS) of the name of a new employer, you must make an request to the NIS office that is part of the Ministry of Labour and Social Security (MLSS). Check out the NIS contact details under the “Office Contacts and Locations” section further down.
  2. Request the application form at the NIS office in person, or download the application form via the MLSS website by clicking the following link:
