Apply For Replace Lost or Damaged Yellow Health Card in Denmark
If you’ve destroyed or lost the Yellow Health Card you apply for a replacement following these steps:
- If you’ve damaged or lost your card or damaged it, you can purchase another card online. you’ll need an NemID for the application.

- An updated health insurance card costs DKK 200. You can pay using Dankort and Visa.
- It can take up to 4 weeks for your card to be sent to you. If you don’t receive your card within four weeks, you can make contact with the Citizens Service Center within 60 days. After 60 days, you’ll have be prepared to cover an extra cost.
Required Documents For Replace Lost or Damaged Yellow Health Card
To get an CPR number, you’ll have to show proof of your address and identification:
- Proof of residence/Housing confirmation
- Rental Contract. The contract must be signed and date. You cannot apply for registration and obtain the CPR until you actually reside in your home.
- If you reside in a residence offered by the employer. The proper documentation is required for this. Contact your employer if lacking this document.
- Hotel payment receipt
- AirBnB or similar accommodation Booking confirmation. Request the owner to fill out the housing confirmation/proof of residency form.
- Passport
- National ID
- Birth Certificates – documents for families
- Residence Permit
- EU Registration Certificate
- Certificate of Marriage Certificate
- Registered Partnership Certificate

A brand new Yellow Health Card DKK 200. It is possible to pay using Dankort as well as Visa.
Validity
The validity of your health card is for as long as you’re living in Denmark and there aren’t any modifications to the information included on the card.
If you plan to travel within the EU you must have an insurance policy with blue stripes.
The Document is required
The yellow health card (Sundhedskort to Danish or “the yellow card”) gives you access to medical care in Denmark.
