Required Documents For Replace Lost Work Permit

Valid passport
Copy of the missing Iqama If there is one
2 recent colour photos (4 x 6 cm in size)
Affidavit/Statement from the police detailing the reason why the permit was broken or destroyed.
Copy of the permit that was lost (if there is one).
Cover letter to regional office.
