Proposal for Registration Provisional

  • The Form 4 is part of the Medical Act 1971 for Provisional Application for Registration.
  • Completed Appendix A Form
  • 2 passport size photos
  • The Deans original letter or a certified copy of the Basic Medical Degree
  • A true and certified document that contains your entire the training (Local students from public universities are exempt from this).

Request for full registration Medical Practitioners undergoing Housemanship in Malaysia:

  • Formula 6 from the Medical Act 1971 to be completed by the hospital.
  • The form 9 under the Medical Act 1971 to be completed by the person applying (medical official).
  • Appendix A form that must be filled out by the applicant (medical agent).
  • Form A should be filled out by the appropriate supervisors of discipline where the housemanship was carried out (including extensions letters where applicable).
