1. Get the Police Report

  • Start by going to an officer station to report any destruction or loss of the dental registration certificate general. You must obtain an official police report from police department, which is the prerequisite for applying.

Step 2 Apply online

  2. Log in to KMPDC Online Services Portal. Log in to KMPDC Online Service Portal by using your “Username or email address” as well as your password.
  3. When you are on your account’s homepage, click on the “Registration ” tab within the menu in the left pane to access your next screen.
  4. On the next page , click next to click”Replacement” on the next page. “Replacement “option to complete an application for replacement.
  5. On the replacement page , click at”Click Here to Make Replacement of Registration Certificate” or the “Click Here to Make a replacement of Registration Certificate”button to go to the next page.
  6. Attach the police report you have obtained, and then click on the “Save & Continue” button.
    • The result will be an invoice for the payment of the fees for replacement applications to be issued.
  7. On the page for MPDC invoices “Click to Pay Now” Click to pay for the replacement of your Annual General Dental registration certificate..
    • Pay the bill by following the instructions online. Instructions for payment will be automatically generated and displayed within the portal. Additional information on the replacement fees that must be paid can be found on section “Fee” section on this page.
  8. When you make the payment for the replacement an invoice for the payment and an invoice for the Replaced General Dental registration certificate will be issued. General Dental registration certificate will be issued on the internet.
    • Note Note: Wait about 10 minutes before refreshing the page for invoices. When you refresh the invoice page you will see the receipt and the license ready to print.
  9. Click here to open the “Download Receipt “ button as well as the “Download Certificate “ Click here to download the payment receipt as well as the replacement annual General Dental registration certificate respectively.
    • Print the receipt as well as the replaced registration certificate, and save them as a reference.
