Eligibility
- Attained the age of 12 years old but is not older than 16 years old.
- It is mandatory that the sponsor be present, and is the
- Parent or
- Guardian (the guardian could be grandparents, grandfathers, aunts or uncle, the older brother or sister who is older than 21).
Fee
- The cost of it is waived for infants from 12 years old up to 12 years and for a period of 30 days.
- If the application is not submitted within 30 days of the deadline, the application will be subject to a processing cost.
