Apply For Obtain Community Services Card in New Zealand
- To apply for the Community Services Card you can apply for one by calling the number for Work and Income – 0800 999 99.
- You can get an application fill out a form at:
- your doctor
- an apothecary or
- A service centre for work and Income service centre, or
- You can download forms on the Work and Income website – click here
- The Work and Income department will email an email to you telling whether you’ll receive an account. If you’re eligible for the program, your card will be included in the letter.It could take up to three weeks for the card to be received. Keep receipts of doctor’s visits or prescription charges, and you will be able to claim them back at a later date.

What are all the Eligibility
You may apply for the Community Services Card if you are:
- at minimum 18 years old (or at minimum 16 and enrolled engaged in full-time study in a tertiary institution) and
- If you have a low or middle income (the amount you earn is based on the family situation) and
- A New Zealand citizen or permanent resident who is a resident here.
It is also possible to apply for (or already applied for) protection or refugee status.

Other uses for the Document/Certificate
The Community Services Card can reduce health costs. It will cost less to pay for certain healthcare services simply by showing the card.
The Community Services Card reduces the cost of:
- Full-priced prescriptions
- visits to:
- an after-hours doctor
- A doctor who isn’t your typical doctor
- glasses for children who aren’t yet 16 years old
- emergency dental treatment
- accommodations and travel to receive hospital treatment in a public hospital when you’ve been sent to a different location
Additional discounts are also available to Community Services Card holders such as rebates for insulation in homes or admission to swimming pools.
