Documents that are required Permanent Resident Card

  • The Application for Permanent Resident Cards(fully filled out and signed)
  • Supplementary Identification Form http://www.cic.gc.ca/english/pdf/kits/forms/IMM5455B.pdf
  • One (1) primary identity document
    • your passport, travel document, or
    • the travel or passport document you had when you became permanent resident (including the passport page that was stamped at the time you arrived in Canada and then became a permanent resident) or
    • The certificate of identity and travel documents issued by the Ministry of Foreign Affairs Canada or any other country.
NOTE: The copy should include the type of document and number, the issue and expiry dates as well as your name, photo and birth date.
In exceptional circumstances, if it is not possible to acquire one of these, you must submit a
  • Copy of any identity document that was issued outside of Canada before your entry into Canada or
  • A statutory declaration that you have signed attesting to your identity , and an additional statutory declaration attesting to your identity , signed by:
  • A person who knew you prior to your arrival in Canada (such as family members) as well as an official of an organisation that represents people who are from your country of citizenship or a former resident.

Format: Clear and legible photocopy

  • Two (2) additional identity documents
    • your Record of the Landing (IMM 1000) or Confirmation of Permanent Residence (IMM 5292 or IMM 5688) or
    • Your valid provincial driver’s license or a valid provincial photo identification card, or
    • Your current University/College photo student card , or

  • Additional documents to prove of residency in Canada within the last five (5) years

Include the following information in your application:

  • Photocopies of every page of your passports that you’ve were issued in the last five (5) years (if the stamps you have aren’t written in English or French you should get them translated, refer to section on translation of documents) and

A duplicate from one or more of these:

  • Other income tax assessments issued by the Canada Revenue Agency within the last five (5) years or
  • transcripts from an accredited post-secondary institution in Canada attesting attendance during the last five (5) years or
  • For all children who are in school for all children in school, records of attendance during the past five (5) years.

NOTE: If you are not able to present two additional identity documents or proof of residency as mentioned above, you must provide an official, dated and signed declaration that clearly explains the reason for the absence.

To be eligible to receive the PR Card You must meet the following requirements:

  • to be permanent residents of Canada

  • A preferred payment method is via the internet.
  • You can pay for your fees on-line if you’ve:
    • A credit card (Visa, Mastercard or American Express)
    • access to a computer that has an Internet connection
    • an email address that is valid
    • access to printing (you will have to create the invoice)

If you don’t want to make a payment via the Internet payments must be made at a bank or financial institution located in Canada. Financial institutions will inform you the payment method it accepts. There is no charge for banking for paying at a bank.
