  1. If you want to apply for a travel restriction document, visit any Immigration Office.
  2. Send the required documents.

  4. Make the payment.

Required Documents For Apply for Restricted Travel Document

  • Completed Im.42-pin form from 1/97. The form must be printed on both sides of white A4-sized paper.
  • Recent photo with light blue background, in the dimensions of 3.5 5 cm X 3.5 centimeters (2 copies)
  • Prior Travel Document (if there is one)
  • Identity Card (MyKad)or Army Identification Cardor Police Identification Card as well as temporary Certificate (JPN.KPPK 10 or 09) for applicants over 12 years old
  • Birth Certificates for applicants who are less than 12 years of age

Restricted Travel Document RM5.00

Validity

A travel document that is restricted is valid for five years and is able to be renewed under the form of endorsement for up to 5 years after the date of expiration.
