  • A authorization letter should be sent to the Consulate with authority over the area.

Documents that are required Travel Permits For Minor

  • The authorization letter must contain:
    • Authorizing name or identification number and the exact address.
    • Name of the child traveling and birth date, location and the reason for the travel.
    • Name and contact details of those who are accompanying the person.
    • or, a signature that authorizes (the signature should be directed to the Consulo).
    • Certificate or proof of the birth of the child.
    • Passport or identity card of the signatory to the letters.
    • Photo of the kid.

Note:

The signature should be taken ? in front of an officer from the consulate.
