Required Documents For Book or Reserve Vehicle Registration Number
- Registration Number Reservation Form In Bids (MV 10) are available at the Office of Road Transport State / Branch or Website RTD (http://www.jpj.gov.my)
- Each application must include a photograph of passport or identity card or birth certificate/receipt in the event that it is valid identification or a Police Identification Card or Military Identification Card of the person applying or a photocopy of a Business Certificate of Registration Certificate (Form D) (Peninsular Malaysia only) or a copy of the Business License (Sabah as well as Sarawak only) or the photocopies of the Form 9 or 13 or a photocopy of the registration certificate or cooperative organization as well as professional body; an official confirmation letter from reservation request registration numbers (for Government / Local Authority or Statutory Bodies / Organization of the United Nations or Embassy or High Commission / International Organization and Office Regional). - Every reservation has to be made with an advance payment equal to 50percent of the total cost of reserving a reservation in full. The advance amount not more than 50percent of total amount is cancelled by itself.
- Draft Bank/Money Order or Postal The order is a deposit placed ? payable directly to DIRECTOR GENERAL ROAD TRANSPORT ASSOCIATION. Personal checks and cash are not accepted.
