- A copy of the completed Form LPS. Form The LPS (JPJK15 pind.1 or 2009)
- Registration Certificate for Vehicle Registration Certificate (JPJK2) Original
- Notes on Insurance Coverage for LPS used
- A duplicate of the Driver’s License
- Copies of PSV Driver License
- The itinerary for LPS that is being used
- Passenger list
- A duplicate of the document bearer’s identification document (if the application is made by a representative of the authorized owner)
