Required Documents For Seaman Identity Book
- The Hong Kong identity card is your Hong Kong identity card
- Hong Kong Seaman’s Employment Registration Book
For those who are not 18 year old applicants The following documents are required:
- If you are married you are married, solely the certificate of marriage will be required.
- If you are not married If not married, the following documents should be presented:
- The parent or guardian needs to complete section 3 of the document (Form ID94) to signify his or their consent to the application
- Hong Kong identity card or any valid travel document from the guardian or parent who is the signatory.
- Birth Certificate(if was signed from the parental) or a Court Order(if was signed by the guardian of the legal custody)

What are all the Eligibility
- Bonafide Seafarer
- A Chinese citizen staying within Hong Kong; or
- An HKSAR resident who is not of Chinese nationality, but permanently resident for a period of indefinite stay in Hong Kong, and is ineligible to get the national visa or passport issued by any other nation or territory.
Fees
The charges for the use of the HKSAR Seaman’s Identification Book (whether of limited or full validity) are in the following order:
- HK$215 (or US$28)
Charge for applications created outside Hong Kong
- Delivery via double registered air mail Hong Kong Dollars (or US$6)
- Delivery via express dispatch service
- Asia-Pacific HK$125 (or US$16.5)
- North America and Europe HK$180 (or US$23.5)
- Other areas of HK$250 (or US$32.5)
Bank handling fee (If the bank draft is not drawn at an institution that is HKSAR or is not drawn within the HKSAR currencies)
- HK$100 (or US$13)
