Dead in Hospital

  1. The nurse will present an official medical record book and hand it to the doctor who is attending.
  2. A doctor can then sign the medical death certificate.
  3. The medical certificate can be delivered into the Civil Registry by a health staff member or handed to the family member who will submit it to the Civil Registry Office.
  4. Once the family has submitted their medical certificates, information will be directly entered into the Death Register (hardcopy) and will be assigned the registry number.
  5. The Registry office will inform the family members of the deceased of when they are able to return to pick up the death certificate.

“Death at Home”

  1. The family may need to make an appointment with a doctor or have the police transfer your body into the medical facility where it will be declared dead.
  2. The doctor who visited the deceased must sign an official medical death certificate.
  3. The medical certificate can be presented for the Civil Registry by a health staff member or passed on to the family members of the deceased.
  4. Once the family has submitted their medical certificates at the Civil Registry Office, details will be directly entered in the Death Register (hardcopy) and will be assigned an identifier number.
  5. The Registry office will notify the family members of the deceased of when they are able to return to collect the death certificate.

The Death of the Island (body was returned in Niue)

  1. The death certificate issued from overseas the location of death will be retrieved by the civil registration staff on the ground at the airport.
  2. The Civil Registry Office will retain the death certificate issued overseas in its the file (with those medical documents for deaths which happened locally) but it is not recorded in the logbook since the death is already registered legally.
