  1. You or your declarant need to visit the Register General Department (RDG). Please view the RDG Contact details and map via this ink Link. The RDG Branch contact details via this link Link. A declarant can be either a Bride or Groom whose marriage record will be amended or a person presenting an attorney or a power of attorney.
  2. Download the application form to correct an error in the marriage record at the RDG office.
  3. Please read the instructions carefully and complete the form in accordance with the instructions. The dates must match those signed by the Notary Public/J.P. The date you sign must match the date you signed. If errors occurred while filling out the form, they must be crossed out by the Notary Public or J.P.
  4. Next, prepare all required documents and attach them with the application form
  5. Pay the application fee at the RDG office closest to you, depending on the service that you are applying for, e.g., express or normal.
  6. Submit the completed application to the RDG official at service desk for processing and verification. Before processing the application, a declarant must be interviewed by the RDG.
  7. After a successful application, RGD will process the request and amend your marriage record in the manner you requested.

A Death Record

  1. You or your declarant must visit the Register General Department to correct or update a death record. Please view the RDG Branch contact details via this link . Note that declarants can be Widow, Widower, or Children or Legal Guardian.
  2. You can either obtain the application form to correct an error in a death record at the RDG office, or you can download it directly from this link: Link
  3. Please read the form carefully and complete it in front of a Justice-of-the-peace (JP) or Notary public. The dates that are signed by the Notary Public/J.P. must be the same as the Declarant’s date. The date you sign must match the date you signed. If errors are made while filling out the form, they must be crossed out by the Notary Public or J.P.
  4. Next, prepare all required documents and attach them with the application form
  5. Pay the application fee at the RDG office closest to you, depending on the service that you are applying for, e.g., express or normal.
  6. Submit the completed application to the RDG official at service desk for processing and verification. Before processing your application, a declarant must be interviewed by the RDG. Interviews for signing Declarants are held Monday through Friday at 8:30 a.m. – 2:00 p.m.
  7. After a successful application, RGD will review your request and amend the death records.

A Birth Record

Please read the form carefully and complete it in front of a Justice-of-the-peace (JP) or Notary public. The dates that are signed by the Notary Public/J.P. must be identical to the Declarant’s date. The date you sign must match the date you signed. If errors occurred while filling out the form, they must be crossed out by the Notary Public or J.P.
  4. Next, prepare all required documents and attach them with the application form
  5. Pay the application fee at the RDG office closest to you, depending on the service that you are applying for, i.e. normal or express.
  6. Submit the completed application to the RDG official at service desk for processing and verification. Before processing the application, a declarant must be interviewed by the RDG.
  7. After submitting a successful application, RGD will review your request and amend your birth record.

Note All Amendments will appear at the bottom on the newly printed certificates. The original recording cannot be removed.

Resident for the Bride or Groom

For the Bride’s and Groom’s Fathers’ Name

  2. Birth Certificate for the Bride’s Father
  3. Groom’s Certificate of Birth
  4. Groom’s father’s birth certificate
  5. Order of the Court (where applicable).

Groom’s occupation and Bride’s occupation

  1. Valid identification, such as a passport that reflects the occupation at the time the marriage was completed or a work identification card

  1. Birth Certificate for the deceased

To correct the gender of the deceased

  2. Certificate of marriage
  3. Birth certificate for children

To correct a deceased occupation

  1. Letter of employment
  2. Job ID

To correct the date of death

  1. Medical Certificate of Cause of Death (MCCD), or Post-mortem examination report

To correct the decedent status

  1. Certificate of marriage
  2. No impediment
  3. Absolute denial
  4. If applicable, death certificate for the former spouse

  1. Document of identification (Passport or National ID, Driver’s License).

Instructions

  • All signatures must be identical to the ID
  • For processing an application, ID must be provided by the Applicant and Declarant.
  • All errors must be corrected and signed by the same signing JP/Notary public
  • All information must be written clearly
  • Except for those issued by Registrar General’s Department, all photocopied IDs must be certified by J.P./Notary Public.
  • A declarant who has an overseas address must sign before a Jamaican J.P.
  • Incomplete applications won’t be processed
