Subscription Application Form convention.
Photocopy RUT, the name and address of the municipality or the institution.
Decree for the nomination of the Mayor or head of the Institution higher.
Identification card photocopy of the official representative of the municipality (Mayor) or the chief of the organization which is the legal representative.

Private entities

The amount that the contract signed Entity is required to pay to provide information services is established through Exempt Resolution No. 2357, dated 1 October 2004 that established the Civil Registry along with Identification.

The values that the Bank will pay for services stipulated in this agreement are:

A replacement connection for the network that connects you to the corporation for the Civil Registry and Identification will cost 2 UF each time you use. All prices must be inclusive of to the value Added Tax (VAT), making the final cost 5 decimal places. The payment will be made in the currency equivalent to the national currency in the amount of the current value in this currency. Unidad de Fomento (UF) up to the date stipulated in the agreement.
