- Original and duplicate from the Identity card
- The original and copy of Birth Certificate or the most up-to-date copy of Birth Certificate
- Original and duplicate from the Passport or any other documents for travel
- Original and a copy of the marriage document
- 3 recent passport-sized images with blue backgrounds

Eligibility
- You must have been born within the Federation (excluding Sabah and Sarawak) prior to Independence Day
- The applicant must be aged 18 or over on the date of application
- Must have been in the federation for the seven years prior to the time of application, for an entire time of at least five years.
- Should have the intention of residing for a long time within the federation.
- Should possess good character
- You must possess at minimum an elementary understanding of Malay language
Fees
- Application fee RM 10.00
- Cost for issuance of Citizenship Certificate (if it is approved) (if approved) – RM 100.00

- Form A (which can be obtained from the Putrajaya NRD headquarters or state NRD headquarters/branches)
- Forms for birth registration in a different country as well as the renunciation of citizenship can be obtained from consulates in foreign countries.
