  3. Download the application form at one of BSJ offices located in Kingstone, Montego Bay, Mandeville, Ocho Rios, or Savanah-La-Ma. You may also get the application form via either email or by phone.
  4. Complete the application form in the manner directed.
  5. Create a test small amount of the pesticide that needs to be tested, along with all the necessary documents.
