Application In Person

  • Application is completed Formula JPJL1.
  • Passport / NRIC (original or duplicate).
  • Certificate for the 6 hour Theory and Practical Course (JPJL2B).

  • Certificate/Test Results Slip for Part 1.
  • 1 color photograph (25 millimeters x 32 millimeters).
