  1. Fill out an application and send it with these documentation in the following documents to Health Authority – Abu Dhabi’s Customer Service Centre:
    1. Book for family members (for UAE Nationals) : Original and copy
    2. Emirates ID Card: Original and a copy
    3. Parents passport: Original and replica
    4. Child’s Passport: Original and duplicate
    5. Birth Certificate Copy, if there is one
  2. Pay the fee required.
  3. Get a replacement certificate for the birth certificate that was lost.
