- 2 copies of the form JPJK1 in regular and full.
- The identification document:
- Shares, sole proprietorships, or sole proprietorships Shareholders: A copy of the original add Business, or other
- Company Sdn. Bhd. or Bhd. or Bhd. certificate of incorporation (Form 9) or Certificate of Incorporation of Change in name (Form 13) or any similar document
- Cooperatives or organizations The photocopy from the certificate of Registration or the Cooperative Societies
- Local Authorities and Government Agencies and Statutory Bodies: A official letter of request to register
- Nations United / Embassy / Office of the Commissioner High International Organization / Body professional Letter of Approval from the Department of Foreign Affairs Negeri Malaysia
- Regional Office: letter of approbation of the establishment offices Regional Ministry of Trade International and Industry (MITI) Ministry of Finance Ministry of Finance
- Sales Letter
- Appropriate Permit (AP) Original and one copy of photostat
- Customs Form Number. 1 and/or 3 , 8 or 9. from the origin
- A photocopy of the RTD Weight Certificate
- The photocopie of document of approval for vehicles using SIRIM green diesel that remains in force (for engines powered by green diesel vehicles).
- The photocopy must be of the T1 Certificate (for Taxis that are metered)
- Puspakom Inspection Certificates (for The vehicle has to go through an inspections on a regular basis)
- Preliminary Exam Report PG11A (for The vehicle has to go through an periodic inspections)
- The Preliminary Examinary Report bas Preliminary Examination Report Bas (for buses that require an periodic inspections)
- Acceptance of Offer Letter as well as Formula Specifications of the Vehicle (for the vehicles to be licensed)
- A report on the Report for the Special Exam B2 (for off-road vehicles)
- Two duplicates of the Form JPJK11 completed by the owner registered (for off-road cars)
