Replace Lost or Damaged Nationality Certificate In Iraq By Online
- To get a new or duplicate national certificate the applicant needs to contact the Ministry of Foreign Affairs.
- The application forms are available from the Ministry of Foreign Affairs.
- The application must be filled out and then submitted at the discretion of the consul.
- If the form is for a defective substitute the original Nationality Certificate shall be attached to the form.
- If you are requesting an unreplaced substitute or replacement, a copy of the fathers ‘ or brothers Nationality Certificate which has been verified through the Embassy as a true Copy must be included.

Required Documents For Replace Lost or Damaged Nationality Certificate
For damaged Card
- Original Original Nationality Certificate
- 4 recent images will be included in the request.
For Lost Card
- A duplicate of the father’s or siblings Nationality Certificate.
- Four recent photographs will be included with the request.
The original copy of applicant’s passport, parents and fathers passports must be included and sealed with the consular seal, and then authenticated by the consul as a true Copy.
Fees
A sum of 2 USD is equivalent to the value of the certificate.
