Replace Lost or Damaged Civil Status Identity Card In Iraq By Online
- For the replacement or duplicate Civil Status Identity Card the applicant must contact the Ministry of Foreign Affairs.
- Applications can be made from the Ministry of Foreign Affairs.
- The application must be filled out and then submitted at the discretion of the consul.
- If the form is to request an unrepaired replacement the original Nationality Certificate shall be attached to the form.
- If you are requesting replacement for a lost one or replacement, an original copy of the fathers or brother’s Civil Status Identity Card , certified at the Embassy as a True Copy must be submitted.

For damaged Card
- Original Identity Card from the Old Civil Status Card
- Four recent images will be included with the request.
For Lost Card
- A duplicate of the father’s or brother’s Civil Status Identity Card.
- Four recent images will be included with the request.
The original copy of applicant’s passports of the father and mother should be tied to the passport, sealed with the consular seal, and then authenticated by the Consular as a True Copy.
