Apply for Funeral Grant Waiver under National Insurance Scheme in Jamaica By Yourself
- To waive your right request a funeral allowance within the National Insurance Scheme (NIS) it is recommended that you make your application to NIS office within the Ministry of Social Security and Labour’s (MLSS) parish Offices at your place of residence. - Get the request for funeral grant waiver application form at the NIS church office in your area or download the form on the MLSS website by clicking the following link: Link (scroll to locate “Funeral Grant Waiver”)
- Fill out the waiver form according to the instructions and in front of an person who is a witness, usually notary public or a Justice of the Peace official as per the instructions in the “WITNESS’ CERTIFICATE and SIGNATURE” part of the application form. Be aware that if the person applying cannot understand or read due to illness, or lack of literacy The application should be signed by a reputable witness on behalf of the applicant.
- After the application form is completed, signed and authenticated in the manner required Go to the NIS office to submit the application in its entirety at the NIS officer at the desk of service
- A NIS official will confirm that your request and provide an acknowledgement acceptance of your application.
- The request for waivers will be sent to the appropriate NIS official for consideration and processing If the application is in line with the requirements of the NIS
- The NIS will examine the request, and if it’s in compliance they will record the request on their records and deprive you of the right to receive the funeral grant and transfer the rights to the person according to the information when you submitted your form. Be aware that the person must also be a candidate for the funeral grant.
