Apply for Name Declaration under National Insurance Scheme In Jamaica By Yourself

  1. If you wish to declare your name in for the National Insurance Scheme (NIS) Jamaica and to declare your name, you must fill out the form for declaration of name at the NIS office of the Ministry of Labour and Social Security (MLSS). Check out the NIS contact information in the “Office Contacts and Locations” section below.
  2. Download the form to apply for a name declaration at the NIS office in person, or take the forms from MLSS website at this hyperlink: Link (choose “Name Declaration” and download the form)
  4. Complete the application in a timely manner and in front of an Justice of the Peace (JP) official. Have the JP signify the JP’s declaration.
  5. Make sure you have the appropriate supporting documents and join them to the application form.
  6. After your application form and all documents supporting it are signed and stamped when required, go to the NIS office and hand in the entire application in person to an NIS official at the front desk.
  7. The NIS will review your application and verify that the application is in line with the standards required for the declaration of name.
  8. After verification, your registered name will be recorded by the NIS
