Apply for National Health Fund (NHF) Cards In Jamaica By Yourself
- To apply in to apply for the National Health Fund (NHF) Card, submit your application completely in the NHF office or to any Card Centre within the parish you reside in. Check out the NHF contact information and Card Centres under the “Office Address and Contacts” section below.

- Complete the application properly and then send it to your doctor who will confirm the application. Fill in the necessary section for doctors.
- Completely fill out the Application to NHF office nearest to you or at any Card Centre
- When you submit the application After submitting the form, NHF officials will check the information you’ve provided to determine if the application has been completed correctly.
- After being verified your application, it will be accepted so that you can be eligible for NHF benefits. Then, you will receive your NHF card will issued and issued to you.
