  • If you’ve been denied your Mobility Parking permit you must contact at the nearest CCS Disability Action branch. CCS Disability Action to let them know that the permit was lost, so that they can make the permit invalid and issue you with a new one.
  • If you want to apply for an upgrade, you’ll need to fill out the application form once more. An option to fill out the application form can be located under the ‘Documents to use. It is important to note that you won’t require the services of a physician to evaluate you once more since they already have your information already.

Cover the cost of replacement, which is $35.00.

Validity

The replacement permit will be valid for the remaining time on that permit. Example: If your previous permit was scheduled to expire in one year, then the new permit will expire after one year.
