Required Documents For Register a Birth 

  • Duly filled in application Form S submitted by the applicant personally or through register/courier mail.
  • Child’s original Birth Certificate or an attested copy of it from JP.
  • Pakistani Passport and National Identity Card of child’s father – please also enclose their attested copies by JP if originals cannot be sent.
  • Pakistani Passport and National Identity Card of child’s mother – please enclose their attested copies by JP if originals cannot be sent.
  • Two photographs of the child – Child’s name written on the back.
  • Marriage Certificate/ Nikah nama of the parents or its attested copy by JP if originals cannot be send.
  • Letter of consent from Parent (if one of them is a foreigner).
  • Self-addressed pre-paid envelop for dispatch of Birth Registration

  • Mark of identification of child, if any

Need for the Document

The child should be registered within six months of his/her birth. In case a child is registered with the High Commission/Consulate General until he/she reaches the age of one year, the parents should apply for his/her NICOP to NADRA, on issuance of which only the registration of the child would be allowed.
