Register for National Insurance Scheme (NIS) In Jamaica By Online

  1. Visit the NIS office to register for National Insurance Scheme. Please see the “Offices and Contacts” section for contact information regarding NIS.
  2. Download the application form to register at any NIS office.

  4. Please read and complete the application accordingly
  5. Send the completed application along with a notarized copy or original document attesting your age to the NIS office of your parish. Birth certificate or passport
  6. If your application meets the requirements, an NIS official will review it and enter your details into their system.
  7. After submitting your application, you will receive a NIS Registration Number.

NOTE:

  • If their proof of marriage is the birth certificate, or if they do not have their married name recorded in their passport, then the married women must submit the original or notarized copy of their marriage certificates.
  • You can also send an authorisation letter and a third party if you are unable or unwilling to submit your application.
